The following forms are designed to help us work together more easily. During the Covid 19 Crisis, you must read and sign all forms with "Covid 19" in the title. You can do this by printing out the form, signing and sending me a picture of the signed document either as an attachement in an email, as a photo from your phone via text or email, or arriving with the signed forms.
